Collaborates with all members of the multi-disciplinary healthcare team to ensure the delivery of high quality, cost effective care, as evidenced by coordinated transitions of care; achievement of select quality outcome metrics; appropriate utilization of health management programs and resources, within the structure of a patient-centered medical home.

Strong clinical background required, RN or CMA experience required.

Two years clinical experience required, specialty-specific experience preferred. Experience in a case management role strongly preferred.

Essential Functions

  • High Risk Patient Management:

  • Collaborate with the provider, healthcare team, and patient/family to evaluate current health status, develop a patient specific care plan to reduce barriers, promote education and self care, secure needed health care and community resources, and avoid inpatient admissions and emergency department visits. Ensure care plan consistency across providers.

  • Communicate with patients regularly to counsel on disease related symptom management, address compliance with care plans, and advise patients on lifestyle choices to improve prognosis.

  • Proactively connect patients to Payor and community health management resources and programs, as patient needs require.

  • Verify chronic disease conditions are listed accurately in EMR, enter chronic disease conditions accurately in EMR.

  • Transitions of Care:

  • Provide follow up on inpatient admissions and emergency department vists; working collaboratively with the provider and healthcare team to establish plan of care post discharge, including follow up appointment in the clinic to prevent unnecessary re-admissions/visits.

  • Tracks quality outcome results for defined population, and shares the results with stakeholders, including the provider, healthcare team, and TIC leadership. Completes chart audits on all patients in the defined population, identifying opportunities for care. Shares the results of the chart audits and care opportunities with the provider, healthcare team, clinic leadership.

  • Collaborates with providers, clinic staff and clinic leadership to identify needs and resolve day to day issues. Serves as a liaison between payors and the provider.

  • Maintains awareness of own safety in carrying out duties and responsibilities. Adheres to universal precautions and infection control policies and procedures.

  • Assumes accountability for enhancing own knowledge through professional associations, journals, workshops, inservices, etc. Keeps professional license current.

  • Maintains patient confidentiality and uses discretion in patient interactions.

JOB SUMMARY

MINIMUM REQUIREMENTS

Education:

Strong clinical background required, RN or CMA experience required.

Licensure/certification:

Current State of Iowa Nursing License, or

Current Certification as a Medical Assistant (CMA).

Previous Work Experience:

Two years clinical experience required, specialty-specific experience preferred.

Experience in a case management role strongly preferred.

Skills:

Demonstrated ability to effectively communicate in English with people from diverse professional,

educational and lifestyle backgrounds both orally and in writing. Ability to read and write legibly and

understand/follow written and verbal directions. Must demonstrate ability to utilize effective decision-

making and problem-solving techniques. Ability to work independently and interdependently with the

health care team. Requires organizational and self-motivational skills in prioritizing and managing

multiple tasks. Must possess skills to operate business office machines, i.e., computer, fax and

photocopy equipment. Ability to remain calm and professional in stressful situations and focused

amid constant interruptions in a fast paced environment.

ORGANIZATIONAL RELATIONSHIPS

Reports to:

Director of Quality and Care Management. Will have daily collaboration with

providers, clinic staff and clinic leadership.

Supervises/accountable for: None.

PHYSICAL REQUIREMENTS AND ENVIRONMENTAL CONDITIONS

Employees are subject to the risks inherent to working in a medical office and hospital environment. This position

functions in a patient care environment and requires:

Extended periods of standing, twisting, stooping, bending, squatting, reaching and ability to bend and lift at the

same time.

Full range of body motion in the handling and lifting of patients.

Ability to lift 50 pounds and lower 200 pounds to the floor; bend to the floor and reach overhead; and crouch, push

and pull in working with patients and equipment.

Manual dexterity necessary for inputting computer data and operation of business machines, i.e., copy and fax

machines, calculators, etc.

Verbal and auditory ability necessary for communicating both in person and via the telephone and participating in

oral conversations with both physicians and patients.

Visual acuity necessary for direct patient care, charting, recognizing outdated medications/supplies and reading

written instructions.

Mental ability necessary for assimilating information and utilizing effective decision-making and problem-solving

techniques.

Emotional demands require working and remaining calm, cheerful and professional in a fast-paced environment. The

environment may cause high stress levels due to interruptions, high volume, urgency of issues, deadlines and

interactions with a wide variety of personalities.

Employees in this job classification have been identified as having the likelihood of occupational exposure to blood or

other potentially infectious materials and are therefore included in the OSHA Exposure Control Plan with all its

specifications for preventing or dealing with contact with the above materials.

NOTICE OF RESIGNATION

This position is a position "at will". However, for purposes of benefit pay out upon termination, three weeks written

working notice is required.
