MDS Coordinator

Position Summary:

The MDS Coordinator is responsible for the timely and accurate completion of the Resident Assessment Instrument (RAI) process. With a thorough knowledge of state and federal regulations, you will provide leadership and assistance in ensuring compliance to requirements.

Required Experience for MDS Coordinator:

  • Must possess high school diploma or equivalent.

  • Must be a Registered Nurse with required state licensure.

  • Significant technical experience and demonstrated leadership/management skills in long-term care required.

  • Geriatric nursing background required.

  • Restorative care philosophy, excellent professional skills (both assessment and hands-on), and good communication and organization skills.

  • Must have working knowledge of the state and federal rules and regulations governing SNF.

Primary Responsibilities MDS Coordinator:

  • Conduct and coordinate the development and completion of the resident assessments and care plans in accordance with state and company

  • Generate Material Data Sets (MDS’s) in accordance with current rules, regulations, and guidelines of the state and the policies and goals of the community.

  • Perform administrative duties such as completing medical forms, reports, evaluations, studies,

  • Develop, implement, and maintain an ongoing quality assurance program for resident assessments and care plans.

  • Assist the resident and Discharge Planning Coordinator in completing the care plan portion of the resident’s discharge plan.

  • Participate in facility surveys (inspections) made by authorized government

  • Monitor all current Springs Residents’ medical records to ensure correctness and completeness of nursing charting, especially in relation to the Plan of

  • Provide immediate supervision and direction to Medical Records Assistant if applicable, in regard to the review of MDS's and Care Plan Input, pharmacy data input, physician Progress Note compliance and other reports completed by the Medical Records

  • Attend Care Plan/Rehabilitation Conferences and participate in Quality Assurance and Ethics Committee

  • Provide and coordinate training on Care Plans with other departments and Springs Nursing

  • Monitor Chemical and Physical Restraint Programs and ensure restraints are reviewed for reduction per Community Policy and State/Federal

  • Monitor Abnormal Involuntary Movement Scale (AIMS).

  • Maintain and monitor infection control and decubitus

  • Participate in giving direct resident care as

  • Report all accidents, incidents or suspected abuse according to

  • Follow established safety precautions in performing all

  • Report all safety violations and take necessary steps to prevent

  • Follow up on all complaints made by Residents, their families, visitors and Associates and report them to the Director of Nursing and/or

  • Develop and participate in the planning, conducting, and scheduling of in-services.

  • Attend and participate in annual OSHA and CDC in-service programs for hazard communication, TB management, and blood borne

  • Attend and participate in advance directive in-service programs.

  • Maintain involvement in community and civic health matters and projects as

  • Act as liaison for the residents, their families, support departments, etc., to adequately plan for the residents’ needs.

General Working Conditions:

This position entails standing for long periods of time. While performing the duties of this job, the employee is required to communicate effectively with others, sit, stand, walk and use hands to handle keyboard, telephone, paper, files, and other equipment and objects. The employee is occasionally required to reach with hands and arms. This position requires employee to effectively present information in one-on-one a small group situation with residents, guests, and other associates. This position requires the ability to review detailed documents and read computer screens. The employee will occasionally lift and/or move up to 25 pounds. The work environment requires appropriate interaction with others. Employee must be able to read, write, and speak fluent English. The noise level in the work environment is moderate. Occasional travel to different locations may be required.
