Wheaton Franciscan Healthcare-Iowa RN - Patient Engagement Advisor in Waterloo, Iowa

The RN is responsible for hospital-based telephone triage calls on our community health line, and answers pager calls for providers after office hours. Through theory-based practice, the RN utilizes critical thinking to assess, plan, implement, and evaluate outcome-based care for a select group of patients by phone, to determine appropriate level of care access. The RN also facilitates communication between physicians, patients, and family members. In addition, will be responsible for supporting the Population Health/ACO initiatives. Also works with members of the physician practice team to provide coordination of care and facilitate efficient use of health care resources. Adheres to MercyOne's performance expectations and performs all defined services and other related duties in accordance with the mission, vision and values of MercyOne. Phone Triage - 12-hour shifts - registry/PRN to work as needed BSN preferred. Minimum of 3 years nursing experience. * Current Iowa RN license, Mandatory Reporter. * High language, intermediate to high math, high critical thinking ability. * Basic computer skills required. Ability to work with diverse groups of people. Ability to work under pressure and with a large degree of independence and accountability.
