POSITION PURPOSE

Under limited supervision, ensures prompt and accurate services for patients, nurses, physicians and other staff. Interprets and evaluates the validity and safety of orders and prescriptions. Monitors the quality of pharmaceutical services provided. Dispenses medications in accordance with legal requirements. Provides information to staff and clients regarding medications and drug therapy. Maintains accurate records. Supervises the work of supportive personnel and verifies the activities of supportive personnel. Adheres to the principles of Service Excellence Guiding Behaviors and the philosophy and mission of Mercy in all aspects of job performance.

ESSENTIAL JOB FUNCTIONAL COMPETENCIES:

1.

Demonstrates the knowledge and skills necessary to provide service/care appropriate to the age of the patients served, in accordance with Mercy standards.

2.

Reviews electronic/written medication orders to determine clarity, appropri­ate dose, schedule, possible drug interactions and contra­indications. Consults directly with the physician, physician/hospice nurse or nursing home, if changes or clarification is needed.

3.

Supervises preparation of patient medications using standard techniques and procedures, including all drugs, prescriptions, controlled substances, IV's and IV admixtures.

4.

Successfully interacts with interdisciplinary teams, administrators, physicians, residents, resident families, nursing directors, nursing staff, as well as state and federal regulations.

5.

Directs and monitors pharmacy technicians in proper packaging, labeling and distribution of medications.

6.

Successfully operates the outpatient computer system and prepares, fills and dispenses outpatient charity prescriptions as needed.

7.

Participates in drug therapy monitoring including evaluation of at least the following: a) appropriateness of route and method of administration, b) potential therapeutic duplication, c) drug-drug interactions, d) drug-food interactions, e) drug-lab interactions, f) drug-disease interactions, g) pharmacokinetics analysis of lab data, h) safety and efficacy of drug therapy, I) cost-effective therapeutic alternatives, j) therapeutic appropriateness of medications and k) assessment of patient diagnosis, medical history and any pertinent physical findings.

Complies with OBRA standards for patient counseling on prescriptions.

9.

Maintains departmental medication inventory levels by checking stock and orders supplies and drugs as indicated. Maintains perpetual inventory system. Maintains invoices in orderly fashion to comply with legal standards.

10.

Consults with and advises physicians and nursing staff on matters pertaining to the selection, procurement, distribution, storing and safe use of drugs.

11.

Maintains records as required by department, state and federal laws.

12.

Researches drug information for physicians, nursing, patients and other health care professionals.

13.

Checks pharmacy stock for outdates and proper amount of stock.

14.

Is familiar with different pricing schemes and charging policies.

15.

Participates in hospital educational programs and community outreach programs as a representative of the hospital.

16.

Recognizes and communicates issues/occurrences utilizing established lines of authority, and assists in identifying ways to resolve variances or unacceptable outcomes.

17.

Actively participates in department meetings, committees, conferences, and in-services.

18.

Is knowledgeable of and actively participates in improving patient safety and reducing risk to patient safety and reducing risk to patients. Complies with Safety and Infection Control Policy & Procedure.

19.

Knows, understands, incorporates, and demonstrates the Mercy and Trinity Health Mission, Vision, and Values in behaviors, practices, and decisions.

20.

Maintains confidentiality of information pertaining to clients, physicians, employees, and Mercy business.

21.

Maintains a working knowledge of applicable Federal, State, and local laws and regulations, Mercy and Trinity Health’s Organizational Integrity Program, Standards of Conduct, as well as other policies and procedures in order to ensure adherence in a manner that reflects honest, ethical, and professional behavior.

Demonstrates general working knowledge of computers and department specific software (i.e. Healthstream, internet, etc.)

MINIMUM QUALIFICATIONS

Requires a Bachelor’s Degree in Pharmacy and licensure in the State of Iowa. If work is performed for Oakland Mercy Hospital, a Nebraska licensure is required within 6 months of hire, with initial and renewal Nebraska license fees covered by Oakland Mercy Hospital Pharmacy department. Must have the ability to work independently and to set priorities, make critical decisions, and respond quickly to emergent situations. Must have basic typing skills and be familiar with computers and their functions. Must have excellent interpersonal and communication skills. Must have a general working knowledge of computers and department specific software (i.e. Healthstream, internet, etc.) Must be comfortable operating in a collaborative, shared leadership environment. Must possess a personal presence that is characterized by a sense of honesty, integrity, and caring with the ability to inspire and motivate others to promote the philosophy, mission, vision, goals, and values of Mercy and Trinity Health.

PHYSICAL AND MENTAL REQUIREMENTS AND WORKING CONDITIONS

Must successfully pass employment physical examination and must adhere to applicable organizational requirements on an annual basis for Employee Health Directives. Must possess the ability to comply with Mercy and Trinity Health policies and procedures. This job is categorized as ""Medium Work"". Has the physical stamina to meet the requirements of this job. Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and /or greater than negligible up to 10 pounds of force constantly. Must be able to handle stress effectively. Must understand and accept the possibility of exposure to environmental elements, internal and external, which may affect the work place, such as infectious disease, chemical and/or chemical fumes, dust, noise, physical injury from an out of control patient, and adverse effects of inclement weather which may occur.
