POSITION DESCRIPTION

POSITION PURPOSE

Under direct supervision of a Registered Nurse, assists in carrying out the phases of the nursing process as appropriately delegated by the Registered Nurse. Performs activities and functions of resident care, which are task oriented, and within the skill level of the Licensed Practical Nurse, in accordance with Mercy Medical Center protocol. Adheres to the principles of Service Excellence Guiding Behaviors and the philosophy and mission of Mercy in all aspects of job performance.

ESSENTIAL JOB FUNCTIONAL COMPETENCIES:

1.

Demonstrates the knowledge and skills necessary to provide service/care appropriate to the age of the patients served, in accordance with Mercy standards

2.

Utilizing the nursing process assists the registered nurse in providing resident care from a holistic approach, specific to the age of the resident population identified within the department/unit scope of service, and in accordance with the department/unit standards.

3.

Demonstrates ability to anticipate/identify physical, psychological, social and spiritual needs of the resident and appropriately reports to the Registered Nurse.

4.

Communicates and collaborates effectively with registered nurses, certified nursing assistants and other multi-disciplinary team members, as well as residents, families and significant others.

5.

Implements and documents appropriated teaching needs to residents/family/significant others as delegated by the Registered Nurse.

6.

Implements and documents appropriated teaching needs to residents/family/significant others as delegated by the Registered Nurse.

7.

Prioritizes and implements appropriated interventions and treatments as delegated by the registered nurse, utilizing the formulated plan of care;

8.

Collaborates with the registered nurse to evaluate and document resident responses to the plan of care, mediations, treatments and therapies (ongoing resident assessments).

9.

Identifies and communicates significant changes in residents status (potential or actual adverse resident occurrences) to the Registered Nurse.

10.

Reports and records aspects of data collection/tasks performed into the residents record accurately and concisely;

11.

Upon completion of Pharmacology and Calculations Tests, administers medications (excluding intravenous administration) safely utilizing acquired knowledge and skills according to the physician's orders;

12.

Ensures that documentation accurately reflects supplies, equipment and services provided.

13.

Performs aseptic treatments/procedures per Registered Nurse delegation and physician order and in compliance with Mercy Medical Center protocol;

14.

Maintains knowledge and demonstrates competency in use of resident equipment;

15.

Maintains and annually updates the Licensed Practical Nurse Skills Performance Checklist per Mercy Medical Center established policy and unit standards;

16.

Recognizes lawful/ethical limitations to his/her role in the nursing process;

17.

Takes responsibility and maintains overall unit organizations and safety, and reports discrepancies to appropriated person in a timely manner within limitations of authority;

18.

Supports and is involved in the applications of CQI principals;

19.

Recognizes and communicates issues/occurrences utilizing established lives of authority, and assists in identifying ways to resolve variances or unacceptable outcomes;

20.

Actively participates in department meetings, committees, conferences, and in-services;

21.

Is knowledgeable of and actively participates in improving resident safety and reducing risk to residents. Complies with Safety and Infection Control Policies and Procedures.

22.

Knows, understands, incorporates, and demonstrates the Mercy and Trinity Health Mission, Vision, and Values in behaviors, practices, and decisions.

23.

Maintains a working knowledge of applicable Federal, State, and local laws and regulations, Mercy and Trinity Health’s Organizational Integrity Program, Standards of Conduct, as well as other policies and procedures in order to ensure adherence in a manner that reflects honest, ethical, and professional behavior.

24.

Maintains confidentiality of information pertaining to clients, physicians, employees, and Mercy business.

Demonstrates general working knowledge of computers and department specific software (i.e. Healthstream, internet, etc.)

Performs other duties and responsibilities as assigned.

MINIMUM QUALIFICATIONS

Requires an active Iowa LPN license in good standing. LPN Supervisor Training required within 30 days. One year of experience is preferred. Is able to communicate effectively with all members of the health care team. Positively promotes the image of Mercy Medical Center. Successful completion of security background check. Basic Life Support Certification is required. Must have a general working knowledge of computers and department specific software (i.e. Healthstream, internet, etc.) Must be comfortable operating in a collaborative, shared leadership environment. Must possess a personal presence that is characterized by a sense of honesty, integrity, and caring with the ability to inspire and motivate others to promote the philosophy, mission, Must practice vision, goals, and values of Mercy and Trinity Health

PHYSICAL AND MENTAL REQUIREMENTS AND WORKING CONDITIONS

Must successfully pass employment physical examination and must adhere to applicable organizational requirements on an annual basis for Employee Health Directives. Must possess the ability to comply with Mercy and Trinity Health policies and procedures. This job is categorized as "Medium Work". Has the physical stamina to meet the requirements of this job. Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and /or greater than negligible up to 10 pounds of force constantly. Must be able to handle stress effectively. Must understand and accept the possibility of exposure to environmental elements, internal and external, which may affect the work place, such as infectious disease, chemical and/or chemical fumes, dust, noise, physical injury from an out of control patient, and adverse effects of inclement weather which may occur.
