The oncology nurse navigator is a professional whose clinical nursing expertise guides patients, families and their caregivers to informed decision-making. The oncology nurse navigator is responsible for coordinating the care and services of the identified oncology patient population across the continuum of care; promoting effective resource utilization; assuming a leadership role within the multidisciplinary team to achieve an optimal clinical outcome; overcoming fragmentation of healthcare services.
Facilitates the coordination of patient care services to assure excellence in patient care and patient flow
Participates in screening processes to identify appropriate clients for cancer coordination and coordinates access to cancer services
Follows patient through care continuum/experience, eliminating barriers to services
Works closely with other healthcare disciplines to ensure timely appointments, results reporting, financial need referrals, communication, patient care and follow-up, makes referrals as needed to other health professionals and community agencies
Provides relevant patient/family education as requested. This may include counseling, health education, and patient advocacy
Serves as a resource to medical center staff in the provision of care for the oncology patient
Facilitates weekly care coordination conferences with appropriate team members
Patient Education & Community Outreach
Participates in educational activities directed at community education and support
Collaborates with other health care providers in developing, implementing and evaluating educational materials for patients and their families
Provides appropriate education for the client, the family, health care team and community support systems regarding cancer care including pre-op and post-op care
Facilitates relationships between physicians, vendors, departments and the oncology program
Participates in professional organizations related to oncology
Data Collection, Documentation & Reporting
Ensures accurate and timely data collection and entry into patient data base
Documents patient interactions into patient chart
Reports outcomes and participates in quality improvement activities
Consistently conforms to high standards of practice
Ensures and enhances facility, safety and regulatory outcomes by maintaining compliance with applicable laws, regulations and accrediting agencies
Education: Graduate of an accredited program in professional nursing. Registered Nurse. BSN or Master’s preferred
Experience: Demonstrated competence of three years of acute care or specialty nursing experience. Demonstrated competence of five years of nursing experience. Medical or Surgical oncology and/or general oncology experience preferred.
License(s)/Certification(s): Registered Nurse. Valid driver’s license when driving any vehicle for work-related reasons. Nurse Navigation certificate perferred.
Knowledge/Skills/Abilities: Knowledge of acute care or specialty nursing. Care coordination or nurse navigation preferred.
