Pay Range: ($25.58-$38.75)

JOB SUMMARY Works to ensure the timely and seamless clinical transition of patients through medically appropriate levels of care. Uses skilled interventions and evidence based practice knowledge to collaborate (patient, physician and multidisciplinary treatment team) and achieve quality outcomes, paces care and negotiates appropriate reimbursement. The role integrates and coordinates utilization management, care facilitation and discharge planning functions.


A. Education

  1. Current and valid license to practice as Registered Nurses in Iowa, BSN or approved BA is required or willing to obtain within three years of employment. Case Management Certification preferred.

B. Experience: Five years Clinical RN experience required Diverse knowledge of community services, resources and programs Previous case management experience preferred.

  • Excellent interpersonal communication and negotiation skills.

  • Strong analytical, data management and PC skills.

  • Ability to establish and maintain working relationships.

  • Ability to work independently and exercise sound judgment in interactions with physicians, payors, associates and patients/families.

  • High degree of initiative and judgment.

  • Ability to work flexible hours.

  • Able to set and organize own work priorities and adapt to changes and interruptions.

  • Mandatory Child and Dependent Adult Reporting Education or completes requirement within 6 months of hire. Renews every 5 years.

  • Must provide proof of Basic Life Support- American Heart Association (BLS Healthcare Provider or HealthStream Heart Code) or American Red Cross via current card prior to employment.

  • Maintains current certification in BLS

  • Must provide proof of Basic Life Support (BLS Healthcare Provider, American Heart Association; or American Red Cross, Professional Rescuer-LIVE class only) via current card prior to employment or within 60 days of hire and maintains current status.

  • Completes Mandatory Reporting: Child and Dependent Adult Abuse requirement within 6 months of hire and renews this requirement every 5 years.
