Qualifications:
Qualifications: Licensed Practical Nurse or Registered Nurse with current license to practice in the state of Iowa. Certified Medical Assistants may be considered based on experience.
Job Requirements:
Help determine what type of aid their clients need
Work with clients and other professionals, such as social workers, to develop a treatment plan
Help clients find assistance with daily activities, such as eating and bathing
Research services, such as food stamps and Medicaid, that are available to clients
Coordinate services provided to clients
Help clients complete paperwork to apply for assistance programs
Check in with clients to ensure that services are provided appropriately
Manages assigned patient load, facilitates dialogue between the clinic staff and referral partners to ensure seamless care delivery and communication from initial referral through each patient's care experience.
Meet or communicate via phone with patients to coordinate care and define resources to ensure safe transition to outside resources.
Obtains pertinent demographic and health information for recommending referrals to ensure appropriate care can be delivered.
Communicates with RFH staff when the patients return to the hospital, either through the Emergency Room or for planned admissions to obtain a resumption of care when the patient is discharged.
Monitors and reports any feedback from referral sources, communities, patients or customers that may positively or negatively affect agency operations.
Works independently but willingly assists other team members as necessary to accomplish clinic goals, meet customer service expectations, and fulfill client needs as required
