Job Summary

What you'll do You'll be a key point of contact for residents, assessing and documenting their well-being, tracking their progress, communicating reports to their physician, staff and family. You'll work with other staff and leadership in regularly monitoring a patient's overall health, and report what you see. You'll help residents maintain an orderly, safe residence and help with programs to improve self-esteem and social well-being. You'll be mindful of privacy, proper reporting techniques and will utilize an EMR.

Qualifications

Graduate of an accredited school of nursing. RN nursing licensure for state of practice is required. Must be certified through AANAC as a Resident Assessment Coordinator (RAC-CT) within 6 month of hire.

Ability to communicate and comprehend.

Knowledge of the RAI/ MDS process; knowledge of basic nursing skills.
