Description:

Position Purpose

Provides skilled nursing care to the organization’s clients in accordance with the plan of care as prescribed by the physician and assigned by the supervising registered nurse. These nursing activities are limited to those in compliance with the state’s/province’s LP(V)N/RNA Practice Act, any applicable licensure/certification requirement, and agency policies and procedures.

What You Will Do

  • Effectively and efficiently plans visit content and schedule based on client capabilities, condition, preferences (when possible), physician orders and available resources.

  • Maintains a working knowledge of applicable Federal. State and local laws and regulations, the Organizational Integrity Program, Code of Ethics, as well as agency policies and procedures and adheres in a manner that reflects honest, ethical and professional behavior.

  • Performs specific treatments and administers medications to implement the medical and nursing care plans.

  • Applies appropriate preventive, therapeutic, and rehabilitative nursing procedures and techniques to clients.

  • Utilizes interview, observation and evaluation in assessing clients and applies nursing judgment, consistent with practice standards, in making recommendations to the primary nurse and client.

  • Reports changes in client condition as appropriate and in a timely manner, to the client’s physician and/or Case Manager/designee and obtains orders for changes in the plan of treatment to respond to the client’s condition.

  • Instructs family members in client care.

  • Assists in care plan revisions as indicated.

  • Participates in care conferences with other members of the health care team.

Minimum Qualifications

  • Active registration for Registered Nurse - Board of Nursing, State of Iowa or Illinois

  • An Illinois or Iowa (whichever is not currently held) Registered Nurse – Board of Nursing, license must be applied for within 30 days of hire and secured within 6 months of hire.

  • Graduate from an accredited or state approved school of practical nursing.

  • Must be BLS certified within 3 months of hire.

  • Must have a minimum of one (1) year experience in an acute care setting preferred.

  • Home healthcare experience preferred.

  • Must possess interpersonal skills sufficient to interact effectively with clients and their families as well as all interdisciplinary caregivers, peers, subordinates and supervisors.

  • Valid driver’s license, current auto insurance and reliable transportation to and from work site are required.

  • Must be comfortable operating in a collaborative, shared leadership environment.

  • Must possess a personal presence that is characterized by a sense of honesty, integrity, and caring with the ability to inspire and motivate others to promote the philosophy, mission and vision, goals, and values of Trinity Health.
