We have afull-timeposition available for aNurse Practitionerin Eastern Iowa. We offer a competitive salary and benefits package. This opening has a$7,500 sign-on bonuspaid out in two increments.This role will focus on providing scheduled services at our locations inDecorah, Waterloo, Cedar Rapids, Iowa City (Coming Soon), Clinton, and Burlington.

Reports To:Chief Medical Officer

Job Summary:Deliver care in an integrated setting. The Nurse Practitioner is responsible for providing all aspects of patient care, including diagnosis, treatments, and consultations. Maintains accurate and current patient records. Will work as a team with our nurses and physicians and will assist in delivering premium care to each and every patient in more than one program or location including medication assisted treatment. Demonstrates commitment to professional growth and competence.

As part of UCS Healthcare, team members will follow all laws and regulations, including those at the Federal, State, and Local level. They will also adhere to all UCS policies and procedures, including patient confidentiality, HIPAA/ 42CFR part 2, and all CARF requirements. Essential Functions:Includes but are not limited to:

Patient Care (50%)

  • Developing treatment plans for acute and chronic diseases
  • Perform initial patient screenings and patient history and physical examinations, including MAT physicals.
  • Diagnose and treat acute infections, injuries and illnesses.
  • Educating and guiding patients on disease prevention and healthy lifestyle habits
  • Understanding the changes in health promotion throughout the aging process
  • Performing diagnostic tests and screening evaluations
  • Managing overall patient care regarding lifestyle and development issues
  • Emphasizing preventative care and disease management
  • Prescribing medications and evaluate their effectiveness.
  • Work with a diverse population of patients compassionately
  • Perform specialized treatments and exams as appropriate.
  • Address preventative care, including tests and vaccinations appropriate for a patient's age, history and other pertinent variables.
  • Dia nose patient conditions using history, physical examination and indicated tests, and prescribe treatment as necessary to address their health concerns.
  • Coordinate services internally with physicians, psychologists, nurses, social workers, pharmacists, counselors, and therapists and utilize support staff as indicated.
  • Analyze test data and determine the need for follow-up appointments and further treatment options.
  • Serve as the resource patients need for ongoing care information, counseling and provide guidance for any patients with critical conditions.
  • Order medication as appropriate
  • Deliver quality care while maintaining confidentiality.

Documentation (40%)

  • Collect, record, and maintain patient information (including the medical history, examination, testing results and treatment plans) in a complete and timely manner with legible and complete documentation. Monitor patients' conditions and progress and reevaluate treatments as necessary.
  • Utilize EHR for documentation and E-prescribing as appropriate.
  • Documents in a timely, accurate and concise manner.
  • Completes and signs all notes in a timely manner.
  • Oversees nursing staff documentation and co-sign if needed.

Training and Education (10%)

  • Attend staffing, QA, and other designated meetings.
  • Maintain all licensures, registrations and permits as required and participate in continuing medical education activities as required and in order to maintain and increase professional standards.
  • Facilitates staff, patient and family decision making by providing educational tools.
  • Perform supervisory duties as requested by the executive director.
  • Participate in agency trainings both as a learner and in ongoing educational health presentations to the staff.
  • Direct and coordinate activities of nurses and other medical staff and provide technical and educational assistance in understanding a patient's medical problems.
  • Provide consultation to clinic staff for improvement in their work performance, which enhances overall quality service delivery.

Minimum Required Education, Skills and Experience

  • Master's Degree in nursing with a valid and unrestricted nurse practitioner license in Iowa and certification as a family practice or mental health nurse practitioner.
  • Iowa ARNP, DEA, and CSA licenses.
  • Ability to work within a team and take direction.
  • Demonstrates effective communication strategies.
  • Strong oral, written, and computer skills.

Preferred Education, Skills and Experience:

  • Be familiar with the EHR medical charting system
  • 2 years experience working with behavioral health or families in a practice setting.
  • Experience Independently providing general and preventative care, conducting check-ups, treating illnesses, ordering lab tests and prescribing medication for children and adults.Experience treating the needs of substance use disorder and transgender populations.

Physical Requirements:

  • A valid driver's license is required (an Iowa driver's license is required within 30 days of hire) and liability insurance or an equivalent means of personal transportation and coverage as regular travel within the state of Iowa will be necessary.

Work Schedule:40 hours a week with a weekly schedule as agreed on by the Chief Medical Officer.This is an exempt salaried position.
