Position Title: RN, HIV Clinical Case Manager

Department: SS-Virology

Purpose: To assist in the overall operation of the HIV Early Intervention program. This position will provide nursing case management for HIV/HCV/PREP. This position will follow up on all HIV referrals, perform face-to-face assessments and reassessments. Will monitor lab tests and immunization schedules. This position serves as a case manager that will help patients access Ryan White Part C services. This position also supports patient retention into the program by utilizing outreach and making phone calls for those patients at risk of non-compliance. Supports and empowers patients through the healthcare continuum to include counseling, problem solving "barriers to care, treatment and medication adherence, healthy life choices and self-help behaviors. Collaboration with the Iowa Department of Public Health and local disease prevention specialists to identify PLWHA as a point of entry. Offers education and testing. Maintains the CARE Ware database for HRSA/State grant requitements. This position provides safe, effective and culturally competent patient care for the clinic patients. Key accountabilities include assessment, planning, implementation and evaluation of care using critical thinking and evidence-based practice, adherence to Professional Practice Standards as defined by ANA; and active participation in quality monitoring and performance improvement activities.

Supervisory Responsibility: Direction: The job requires some accountability for scheduling, assigning or coordinating work. Employees check the quality of work and provide guidance, instruction, training and direction to others. Although the job does not require formal or official supervisory responsibility, the incumbent serves as group leader or acts in an informal "assistant supervisory" or "lead" role. In addition, the employee may be expected to provide information or suggestions on human resources matters.

Materials Responsibility: Limited: Work requires limited responsibility for material resources. Examples of resources could include supplies, equipment, inventories, small budgets, and other similar material assets. The employee has a limited amount of control over these resources. The cost of errors is also limited in terms of damage, waste or financial loss. Problems associated with material resources are not complex. The volume of resources may vary, but the level of difficulty in dealing with these resources is uncomplicated.

Education: Associate's Degree

Field Of Study: Nursing

Special Training: Basic Life Support

Training Preferred:

Licensure/Registration: Registered Nurse in Iowa and Illinois; AIDS certification is required.

Experience: More than 2 years experience required.

Interpersonal Skills: Interaction is with a variety of people inside or outside the organization. Communications are often difficult or stressful in nature. Contact with others involves complex, detailed and often sensitive topics. The job requires a high degree of interpersonal skills to deal with a range of complicated problem situations. Interactions involve gaining the agreement of others.

Working Conditions: There is exposure to moderately adverse and undesirable environmental conditions. There are some health and safety risks. Position may require safety equipment and precautions. The amount of time the employee may experience these moderate conditions exceeds 70% of their work day.
